Patient and Family

The patient and family as rehabilitation team members:

The patient and family are the most important members of the rehabilitation team. It is vitally important that open and honest communication takes place between the patient, family, and team members to assure the success of the rehabilitation program.

Patient and family rights:

The patient and family are entitled to certain rights during the rehabilitation process, that may include the following:
  • knowledge of the disability and rehabilitation plan

  • an equal voice in care decisions

  • equal status as team members

  • opportunities to increase self-care activities and independence

  • opportunities to resume social and community roles

  • information about the family's role in the rehabilitation process

  • knowledge about community resources, support groups, and continuation of care after discharge

  • hope and assurance for the patient's future potential

Patient and family responsibilities:

The patient and family also have certain responsibilities during the rehabilitation process, that may include the following:
  • providing accurate medical information

  • providing support and encouragement for the patient

  • following mutually agreed upon treatment plans

  • meeting financial obligations

  • participating in patient and family conferences, education sessions, and discharge planning

By working together, the patient, family, and team can best set and attain realistic goals for optimal health and function, adequate discharge planning, and, eventually, community re-entry.
