For Your Visit Please Bring:

1. A prescription from your primary care doctor or referring physician stating a diagnosis code and the type of service requested. (For example "speech evaluation for articulation and treat as needed.")
2. Have your physician send a prescription for the desired therapy to the Child Development Center fax at 314-872-3180.
3. Pre-authorization from your insurance company if indicated.
4. Information prior to the visit is helpful so that the therapists can plan what testing materials are needed as well as maximize cooperation from you child since they will be experiencing a new person and environment. Please choose from the following:

• Occupational Therapy
• Physical Therapy and Physical Therapy-Torticollis
• Speech-Language Pathology

Cancellation Policy:

We would appreciate being notified at least 24 hours prior to canceling a scheduled appointment. We realize that there are illnesses and accidents that may occur shortly before appointments which will be handled on a case by case basis. However, we would still appreciate notification.
