Insurance and Referrals

Please call our office before you make an appointment with a specialist. Most health concerns can be taken care of by your family physician and do not require another specialist. If your primary care physician decides a referral is needed, we need at least five working days to obtain the referral or authorization number. Our referral staff will take the necessary information over the phone and obtain a referral for you. If you are unable to reach them please leave a message on voice mail or you may send them an email request.

We will need the following information:

  • Patient name, date of birth and phone number during the day
  • Name of insurance (HMO or PPO)
  • Specialist name, phone number and address
  • Reason for referral

Referrals cannot be given to new patients who have never been seen in our office. Referrals are not given for a problem not previously diagnosed and/or discussed with your family physician. Please allow one week for your referral to be processed.
