your health care needs. We look forward to serving you and your entire family.
To help us provide you with the best possible care, please review the information
on this page. You will find important details about a variety of issues.
Our Specialty
Have you ever wished that one doctor could care for the entire family? Our physicians
are trained to do just that. A family physician is a medical doctor who has
completed three years of training after medical school. Family physicians are
trained in all major medical areas, allowing them to care for you and your entire
family, from prenatal care through care for the elderly.
Our Services
We are prepared to take care of you and your familys primary health care
needs. We provide a full range of services for both children and adults, saving
you the cost, time and occasional confusion of consulting separate physicians.
We specialize in health promotion and disease prevention.
This includes general checkups for infants, children, adolescents and adults
(including well woman exams), prenatal care, immunizations, cancer screening,
smoking cessation, stress reduction, and diet and exercise counseling. In addition,
we offer services in natural family planning.
Urgent problems can be cared for on a same-day
basis. This includes the treatment of lacerations and orthopedic injuries. Family
physicians are trained to perform minor surgical procedures involving the skin.
We are also concerned about your physical, emotional and spiritual well-being.
Counseling is available by two psychologists who are in our office. A social
worker and dietitian also are on staff to help you in our office. In situations
where complex problems require specialized care, we refer you promptly to the
appropriate specialist.
Your Primary Care
Our philosophy in family practice is to have one physician care for the majority
of the health care needs of your entire family. We will make every effort to
help you see your physician when you want. However, when your physician is not
available, a faculty or resident physician will be available to assist you.
If you are seen by a resident physician, your care will be coordinated by the
faculty physician supervising the office that day.
Who We Are
Seven board-certified faculty physicians, two licensed psychologists, and a
clinical pharmacist maintain an active practice. They also provide teaching
and supervision to approximately 20 resident physicians and two resident pharmacists
who have chosen to continue their post-graduate education in the specialty of
family medicine.

Monday & Wednesday: 8 a.m. - 4:30 p.m.
Tuesday, Thursday & Friday: 9 a.m. to 4:30 p.m.
The phone lines are open from 8 a.m. to 4:30 p.m., Monday through Friday. Same-day
or next-day appointments are available.
Insurance and Fees
We participate in most insurance plans and accept Medicare and Medicaid assignment.
Please call the office if you have any questions as to whether we participate
in your particular plan. We will be happy to file your insurance for you, but
require you to bring your insurance card and co-payment on each visit. Anytime
you change insurance plans, we ask that you let us know. All co-payments
must be paid at the time of service. Our office accepts payment by MasterCard
or Visa, in addition to cash or check.
How To Contact Your Physician
DURING THE DAY: To reach your physician,
make a future appointment, or schedule a same-day urgent appointment, please
call 569-6010. Our phone lines are open from 8 a.m. to 4:30 p.m. daily. When
necessary, the receptionist will take a message and arrange to have the nurse
or doctor return your call.

Prescription refill: Please have your pharmacy fax over the request. Our fax
number is 251-8889.
AFTER HOURS: For all physicians, please
call 388-6502. Please leave your name, phone number and your physicians
name. If your physician has not responded in 15 minutes, please call the answering
service again.
IN CASE OF EMERGENCY: The best care is comprehensive
medical care. Routine health care and treatment for most illnesses may be arranged
by contacting our office. If you have a life-threatening problem that needs
immediate care, please report to the St. Johns Mercy Emergency Room. The
emergency department staff will contact your physician, who will be notified
regarding your treatment.
Referrals
Please call our office before you make an appointment with a specialist. Most
health concerns can be taken care of by your family physician and do not require
another specialist. If your primary care physician decides a referral is needed,
we need at least five working days to obtain the referral or authorization number.
Our referral staff will take the necessary information over the phone and obtain
a referral for you. If you are unable to reach them please leave a message on
voice mail or you may send them an e-mail request.
We will need the following information:
- Patient name, date of birth and phone number
during the day
- Name of insurance (HMO or PPO)
- Specialist name, phone number and address
- Reason for referral
Referrals cannot be given to new patients who have
never been seen in our office. Referrals are not given for a problem not previously
diagnosed and/or discussed with your family physician. Please allow one week
for your referral to be processed.

Our goal is to provide you and your entire family with quality, compassionate
and timely medical care, as well as exceptional service. If we are not meeting
