Anticoagulation Management Service
| This service is commonly known as a “Coumadin Clinic,” and is run by a clinical pharmacist or Doctor of Pharmacy (PharmD). The pharmacist will monitor your blood levels and make any necessary dosing adjustments. All the information that the clinic obtains will be sent to your physician. The procedure is quickly and easily done with a finger stick. Most insurance plans accepted, and a physician referral is required. |

Advantages of the Clinic

- Only a finger stick, not a venous stick.
- Making appointments is quick and convenient.
- Test results are ready within one minute.
- When a test is abnormal, your doctor will be notified.
- The pharmacist will provide information on drug-to-drug and drug-to-food interactions.
- On the first visit, you will register in Suite 100 of the St. John’s Mercy Medical Building. From there, the admitting clerk will direct you to Suite 154. At subsequent visits, you may go directly to Suite 154.

How the Clinic Works

- Bring a list of ALL the medications you are currently taking, so the clinical pharmacist can review the list.
- The clinical pharmacist will provide information about Coumadin (warfarin) as well as other anti-coagulants or blood thinners you are receiving.
- The clinical pharmacist will test your blood by using a finger stick and make any dosing adjustments.
- Your next appointment will be scheduled based on the results of your blood test.
- The results of your Prothrombin Time (PT) and International Normalized Ratio (INR) tests and any dosage adjustments will be sent to your physician’s office.
